What the assessment is

Before nursing supports begin, a registered nurse visits the participant at home to understand the clinical need, the environment and the goals, and to agree a safe plan. It's a conversation and a clinical review — nothing to be anxious about.

What helps to have ready

• The participant's NDIS plan (or plan summary) and how nursing/clinical supports are funded.
• A current medication list.
• Relevant health information — diagnoses, recent hospital or GP letters, wound or clinical details.
• Details of the GP and any treating specialists.
• Current supports in place (support workers, other providers, informal carers).
• The participant's goals — what "good" looks like day to day.
• Any access notes (parking, keys, best times, communication needs).

Don't have everything? That's fine — bring what you have. The nurse will help identify anything else needed.

What the assessment covers

The RN reviews the clinical need and risks, checks the support sits within the participant's plan and Carehaven's registration, and drafts a support plan — the type and frequency of nursing visits, any competencies or protocols required, and how it links with other supports.

What happens next
