As a referrer, you can send a referral directly to Carehaven — we respond promptly and sensitively, check eligibility and arrange an RN assessment. Clinical decisions about care remain with the treating team.

Short answer: confirm there's a clinical need, complete the DVA referral form (D9389), and send it directly to your chosen provider. DVA doesn't allocate clients — the referral goes to the provider you nominate, and care can start once referral and eligibility are in place.

What DVA Community Nursing can cover at home

Short answer: clinical nursing care delivered in the veteran's home — including wound care, medication support, diabetes management, continence, catheter and stoma care, palliative support and post-discharge nursing. Personal care can also be provided following clinical assessment.

What happens after you refer a veteran — so they won't fall through a gap

Short answer: once your referral reaches Carehaven, we confirm it the same business day, check eligibility and that the need fits our services, contact the veteran, arrange an RN home assessment, agree a care plan and schedule visits matched to need — keeping you informed along the way. Care can commence under the DVA standard Schedule of Fees once a valid referral and eligibility check are in place, with no separate DVA approval needed to begin.

Continuity of nursing for a veteran after a hospital stay
